For patients with chronic medical conditions such as Asthma, COPD, Epilepsy, Severe Mental ill health, Cerebral-vascular disease, learning disabilities and Chronic Kidney Disease.
6-monthly Medical Review - For patients with Coronary Heart Disease, Diabetes and Hypertension.
Asthma
Annual review of medication, symptoms, medication and management. Annual Peak Flow Measurement. If your symptoms are deteriorating or you have any concerns, please make an appointment with the respiratory nurse or doctor.
Important: Please bring your inhaler to your Asthma review.
COPD
Annual review of medication and lung function.
Coronary Artery Disease
Annual review of blood pressure, cholesterol, kidney function and general well-being.
Dementia
Annual review of health status
Depression
Regular review of the depression and medication.
Diabetes
6 monthly blood tests to check diabetes control / Annual review of blood pressure, kidney function, circulation and sensation with one of our specialist Diabetic Nurses / Annual eye test (arranged via the Salisbury & North Hampshire Diabetic Eye Screening Programme).
Hypertension
6 monthly blood pressure / Annual blood test - Please use the following form to submit your home blood pressure readings:

Annual review of health status and/or a 4-monthly blood test if taking Lithium medication

Immunisations
We can provide the full range of child, occupational and travel vaccinations. Some vaccinations are covered by the NHS, but for others a fee is payable. For Travel immunisations, please make an appointment with the practice nurse at least 8 weeks before our intended departure. We are a registered Yellow Fever centre.

Child Health
Child development checks are organised by the Health Visitors. Please contact the Health Visiting Team on 0300 303 1880 who will advise when the checks are due and the days and times of the Baby Clinics.
There are no set clinic times for child immunisations at either Odiham Health Centre or Old Basing Health Centre; invitations to attend for immunisation are sent from the Child Health Authority and parents/carers can then contact either the Odiham Health Centre or Old Basing Health Centre to arrange a mutually convenient appointment.
Minor Surgery Services
We offer a variety of minor surgical procedures, including removal of small skin lumps and joint injections
Contraceptive Services
All the doctors provide a confidential family planning service.
A range of free, confidential services and advice is also available at your local Sexual Health Clinic. This includes:
- Sexually transmitted diseases
- Contraception (Coil, Implant & Contraceptive Pill)
- Emergency contraception ('morning after pill')
- Pregnancy testing
- HIV testing and treatment
- Free condoms
- Vasectomy
- Psychosexual counselling
- Sexual health promotion

Cervical Cytology ('smears')
The national recall system is for all women aged between 25 and 65 years; you will be sent a reminder from them when your smear is due. This is currently every 3 years from age 25 to 50, and every 5 years from age 50 to 65.
This screening program can only be performed upon receipt of your reminder letter from the Health Authority, please make an appointment with the Practice Nurse once received.
Your results will be sent to you within approximately 4 weeks, directly from the laboratory if they are normal. Should your test result require action before your next routine smear recall, the practice will write to you.
