New Patient Registration
Please fill out a registration form, available at one of our practices.
Or, please complete the application form on-line, you will still need to come to the practice to sign the document.
Although we can register you straight away, please ensure you have enough medication from your current GP to last you a few weeks.
You do not need to book an appointment to see your new GP or Nurse as part of the registration process. Your records will be transferred from your previous practice for us to access when you next require medical attention. You will be assigned a 'Registered GP' but you can book an appointment to see any GP within our practice, at either Old Basing or Odiham.
If you are a UK resident, you are entitled to be seen anywhere in the country as an NHS patient if you require urgent medical attention whilst away from home. You will need to register as a temporary patient at a local surgery.
UK residents can make appointments to be seen as NHS patients once a temporary registration form has been completed. Proof of identity will be required and if no face to face appointments are available on the day, you will be offered a telephone call from a GP or the next available appointment.
If you require a repeat prescription, you can contact your registered GP to have your prescription sent via EPS (Electronic Prescription Service) to a pharmacy local to where you are staying.
Overseas residents will be treated as private patients unless they have documentation and proof of identity to confirm entitlement to NHS services. Details of the applicable fees are available in the 'Private Patient Fees' tab.
Practice Catchment Area
We have an open list and accept any patient resident in any of the following villages:
- Crondall (Mill Lane)
- Hartley Wintney
- Hook Common
- Long Sutton
- North Warnborough
- Old Basing
- Powntley Copse
- South Warnborough
- Up Nately
- Upton Grey
- Weston Corbett
- Western Patrick
Unfortunately, we are unable to register patients outside of these areas due to high demand, if you have moved to an area not listed above you wil need to register with a practice closer to you.
Change of Address
Please inform the receptionist of any change in your name, address or telephone number.
Please also remember to inform any hospital that you may be under the care of.
